In addition to office appointments, I charge this amount for other professional services you may need, though we will break down the hourly cost if we work for periods of less than one hour. Other services include report writing, telephone conversations, consulting with other professionals with your permission, preparation of records or treatment summaries, and the time spent performing any other service you may request. If you become involved in legal proceedings that require my participation, you will be expected to pay for all of my professional time, including preparation and transportation costs, even if I am called to testify by another party.
You, and not your insurance company, are responsible for Full payment of fees. You will be provided with a HCFA-1500, also known as a CMS-1500, which you are responsible for submitting to your insurance company for any reimbursement. Please note, many insurance companies now have deductibles, so it is important for you to be aware of how much of your deductible you have met prior to seeing me. Each month, you will receive an invoice containing all the required information that can be submitted to your insurance company for reimbursement using your out of network benefits. The invoice will be accompanied by instructions on how to submit it to your insurance company. However, you (not your insurance company) are responsible for full payment of fees.
