PHONE CALLS:

Dr. Gray is often not immediately available by telephone. While she is usually available between 9AM and 5PM, she will not answer the phone when she is with a patient. The telephone is answered by an answering service that is monitored 24 hours a day seven days a week. Dr. Gray will make every effort to return your call on the same day you make it, with the exception of weekends and holidays. If you are difficult to reach, please inform Dr. Gray of some convenient times when you will be available. Please do not leave phone numbers for Dr. Gray to return your call if you would not want Dr. Gray to identify herself to someone who answered the phone (family member, roommate, etc.). Patient requested phone calls, that are unrelated to scheduling an appointment, may be charged a fee if the time is over 10 minutes.

CONTACTING ME:

If you are unable to reach me and feel that you can’t wait for me to return your call, you may:

  • contact another mental health professional if you have one (psychotherapist, etc.)
  • contact your primary care doctor
  • call the Resolve Crisis Line at: 412-864-5065
  • go to the nearest emergency room and ask for the psychologist/psychiatrist on call
  • in the event of a life-threatening emergency call 911

PAYMENT:

Payment in full is required at the time of service. All patients are required to have a credit card on file. Please see the “Professional Fees” section for further details.

ADMINISTRATIVE FEES:

I require a credit card on file that would be billed for services provided outside the scheduled appointment. In addition to office appointments, I charge for other professional services you may need. Other services include report writing, telephone conversations or other forms of consultation with other professionals regarding your care, preparation of records or treatment summaries, reviewing records from past providers, filling out disability forms and FMLA paperwork, and the time spent performing any other service you may request. There is no charge to send a copy of your medical record to another provider.

CANCELING AND MISSING APPOINTMENTS:

Missed appointments will be charged the full fee, which is NOT covered by your health insurance plan. Cancellations less than 24 hours prior to the appointment will also be charged the full fee. If you need to reschedule your appointment and give more than 24 hours-notice, your appointment will be rescheduled at a mutually convenient time.

LATE ARRIVALS:

Patients are seen by appointment. If you arrive late, the appointment must end as scheduled and you will be charged for the full amount of your scheduled visit. This will allow me to see each patient when they are scheduled.

PRESCRIPTION REFILLS:

My standard of care is to see my patients at least every 6 months. However, I am aware that there may be acceptable reasons why this may not be possible and am more than happy to discuss these with you. Once a stable medication regimen has been established, you will be given a 6 month supply of all medications (other than stimulants or other controlled substances). If you are in need of a medication refill, (other than a stimulant or other controlled substance), please call your pharmacy and ask them to fax a refill request to 412-404-2097. Also, please allow 48 hours for your refill to be sent to the pharmacy. Should you either miss or cancel a scheduled appointment, there will be a charge for me to call in refills. Refills will not be called in on weekends.
